It usually hints at the smoke monster apparitions or the presence of the monster. I can't remember what episodes, but it has definitely occurred once or twice. I just thought it was very interesting how in this episode immediately after Emily's mother is about to light one up the nurse informs her that she can't and then who comes into frame? Alpert. I just can't help but feel this was important, and it seems to me that he is the most probable candidate for being directly connected with the monster somehow.
